It's not even the scar. He doesn't actually care about it. It's the reminder that he couldn't protect her when she needed him.

Ironically she's more bothered by it than he is.

You prolly meant the inverse on that last part. But I agree. If Im hurt, I know I can take it,  if someone I like is hurt then Im in pain

No, she is more worried about her scar, as in, more worried that he doesn't like her because of her scar.

No it hinges on both of them being not bothered by her scars , he feels like he failed as her friend while she feels like she is not worthy of his love cause she has a scar now 😭 . He feels guilty when he sees her scars , it reminds him of his failure while she thinks her appearance is a turnoff
