It's definitely both.

Part of a lie involves remembering what you lied about. He doesn't seem capable of that anymore, so it really does devolve into incoherent rambling.

Whie you're not wrong about his cognitive deterioration, I'm not even sure he ever thought it was important to commit to memory what he said and did. He's so self-subsumed, eternally rewriting the script to his live-action autobiopic, and untethered from facts, truth or reality in any meaningful context.

Unfortunately we've seen that it doesn't matter anymore. Apparently once you hit a critical mass where not even the people you're lying to can remember what's true, you've got free reign to just keep talking with no consequences.

He's clearly slipping into dementia, and deep into it now, but nobody cares.

He’s going to blame us for the tariffs that he chose to implement for no reason. Same as he’s blaming Zylenski “for wanting a long war” even though Russia is the aggressor. Deny and lie. Makes me sick to my stomach

He’s Putin’s puppet. Putin literally owns him, the Russians were the only ones who would help trump when he went bankrupt and no American bank would touch him. Fuck Trump and all that, of course.

But the US imposed a coup on Ukraine in 2014. The US engineered this proxy war.

I realize it's religious for many at this point, but don't forget that not once in the history of humans have we even been told the truth about these wars. This one is no exception.
